Likelihood: 1 Rare, 2 Unlikely, 3 Possible, 4 Likely, 5 Almost certain. Impact: 1 Minor, 2 Low, 3 Moderate, 4 Major, 5 Severe. Score = likelihood × impact (1 to 25). Higher scores indicate higher priority.

How to use this risk register template

Start with four risks and add up to 40. Each record has a description, likelihood, impact, score, owner, response, and status. Remove unused records before printing. Long entries can add pages.

Rating scale

This template uses a general 1 to 5 scale for likelihood and impact. Choose both ratings to calculate a score from 1 to 25, or leave both empty. Agree on what each rating means for your project before you use it. The score helps you compare risks; it does not measure probability or prove compliance with a standard.

Choose Letter or A4 in Settings. Use Print to include your entries. Print blank form prints empty fields and keeps your entries on this page. To save a PDF, choose that option in your browser's print dialog.
